FACILITY FAILED TO PROVIDE APPROPRIATE TREATMENT AND CARE ACCORDING TO ORDERS, RESIDENTS PREFERENCES AND GOALS.

State Findings:

Based on observation, interview and record review the facility failed to ensure residents received treatment and care in accordance with professional standards of practice, the comprehensive person-centered care plan, and the residents’ choices for 1 of 9 Residents (Resident #1) and from 1 staff member (CNA A) reviewed for quality of care in that:

CNA A used Resident #1’s arm to pull the resident back into position to move Resident #1, which resulted in a right arm fracture.

This failure resulted in the identification of an IJ on 7/7/21. While the IJ was removed on 7/9/21, the facility remained out of compliance at an actual harm that was not immediate jeopardy with a scope of isolated due to the facility’s need to monitor the implementation and effectiveness of its POR.

This failure placed residents at risk for improper care, injury, pain, decreased quality of life, worsening health conditions, and even death.

In an interview on 7/7/21 at 1:27 p.m. the Administrator confirmed CNA A reportedly grabbed Resident #1 under his right arm to reposition him in bed on 6/8/21 which led to the finding of a fracture to Resident #1’s right upper arm. The Administrator revealed the expectation was staff were never to use resident’s arms or legs for repositioning and CNA A had been checked off as competent.
