State Findings:
The Department of Health & Human Services conducted an inspection of the facility. The following highlighted decencies listed below were found in a public survey.
Ensure that a nursing home area is free from accident hazards and provides adequate supervision to prevent accidents.
Based on policy review, Director of Maintenance job description, User’s Operational Manual, maintenance supervisor’s log documentation, observation, interview, and medical record review, the facility failed to ensure the environment was free from accident hazards when dangerously hot water temperatures were measured from 126.1 degrees Fahrenheit (F) to 130.6 degrees in 10 of 60 resident rooms (room [ROOM NUMBER], #110, #111, #112, #115, #119, #126, #129, #200 and #210) and a sink in the East Hall shower room. The dangerously hot water placed 9 physically and/or cognitively impaired residents (Resident #15, #17, #23, #24, #27, #48, #67, #72, and #241) that resided in the affected rooms and able to access the hot water, and 10 residents assessed for wandering (Resident #3, #18, #30, #53, #57, #65, #66, #69, #71 and #76) at risk of serious bodily injury, harm, burns, or death.
Have a registered nurse on duty 8 hours a day; and select a registered nurse to be the director of nurses on a full time basis.
Based on facility policy review, review of facility working schedule, daily staffing posting, and interview, the facility failed to ensure Registered Nurse (RN) coverage 8 consecutive hours a day, 7 days a week, for 11 days ranging from May 13, 2023 through June 26, 2023. The facility’s census was 92.
