State Findings:
Based on interview, record review, and review of the facility’s policy, it was determined the facility failed to ensure one (1) of six (6) sampled residents (Resident #4) was free from physical abuse.
On 03/09/2021, Respiratory Therapist (RT) #1 overheard Resident #4 loudly cussing and calling the housekeeper derogatory names. In response, the Respiratory Therapist went to the resident’s room in anattempt to calm the resident. The resident hit the RT and the RT hit the resident back. The Respiratory Therapist identified his response to the resident was reflexive, and he swung his opened-hand back.
The facility transferred Resident #4 to the emergency room , later on 03/09/2021, for an evaluation related to the incident. The Radiology report revealed Resident #4 was diagnosed to have mildly displaced bilateral (both sides of nose) nasal bone fractures (broken nose) with associated hemosinus (blood in the sinuses).
Telephonic interview with State Registered Nurse Aide (SRNA) #6, on 03/17/2021 at 2:34 PM revealed he worked, on 03/09/2021, but was not assigned to Resident #4. He stated the DNS directed him to assist with Resident #4’s change of clothing and linens, and wash him/her up. SRNA #6 stated Resident #4 had blood coming from both sides of his/her nose. He stated the resident’s nose and eyes were discolored, purple, and swollen. SRNA #6 stated he provided the resident with towels, and changed part of the linens as the resident allowed. He stated the resident refused to allow him to change his/her clothing.
