State Findings:
Based on observation, record review and interview, the facility failed to ensure a resident in a wheelchair was properly secured on the transport van to prevent an accident with injury for 1 (Resident #1) of 1 sampled resident who was transported on the facility van in the past 30 days. This failed practice resulted in past noncompliance at the level of Immediate Jeopardy, which caused or could have caused serious harm, injury, or death for Resident #1, who tilted backward in the facility van during transport and hit her head, and had the potential to cause more than minimal harm to 15 residents who were transported via wheelchair on the facility van in the last 30 days, as per a list provided by the Administrator on 09/01/21.
On 9/01/21 at 7:55 a.m., the surveyor drove from the facility to the [CONDITION(S)] clinic to check the distance; the trip odometer indicated a distance of 10 miles (distance resident was transported after the van incident without being checked by a nurse).
