State Findings:
The Department of Health & Human Services conducted an inspection of the facility. The following highlighted decencies listed below were found in a public survey.
Protect each resident from all types of abuse such as physical, mental, sexual abuse, physical punishment, and neglect by anybody.
Based on observation, interview, and record review, the facility failed to ensure residents were free from neglect for 4 of 12 residents (Residents #1, #2, #3, and #4) reviewed for abuse and neglect.
The facility failed to access contraindications of alcohol for Residents #1, #2, #3, and #4 who all had
diagnoses and behaviors that could be intensified by using alcohol use. Residents were provided alcohol during a facility hosted event that led to Resident #1 having a physical altercation with staff.
The facility failed to to adhere to their alcohol policy by not educating residents about Happy Hour. Per the facility’s policy, they failed to montor residents after Happy Hour.
Develop and implement policies and procedures to prevent abuse, neglect, and theft.
Based on observation, interview, and record review, the facility failed to implement their written policies and procedures for prohibiting and preventing neglect and investigating allegations of neglect for 1 (Resident #23) of 4 residents reviewed for neglect. The facility failed to implement their policies and procedures for investigating and reporting neglect after Resident #23’s wheelchair tipped backward causing her to bump her head and shoulder while being
transported in the facility’s van.
Timely report suspected abuse, neglect, or theft and report the results of the investigation to proper authorities.
Based on observation, interview and record review, the facility failed to ensure that all alleged violations involving abuse and neglect, including injuries of unknown source, were reported immediately, but not later than 24 hours if the events that cause the allegation do not involve abuse and do not result in serious bodily injury, to the administrator of the facility and to other officials including to the State Survey Agency in accordance with State law through established procedures for 1 (Resident #23) of 4 residents reviewed for neglect.
