BOULDER, CO- 16 Harm-level deficiencies were cited

DENVER, CO- Copy of the coroner’s report documented resident died from force trauma to the head due to an accident at the nursing home. Director of Nursing said, “I don’t see that we did anything after his first fall”.

LAKEWOOD, CO- Director of Nursing said facility is admitting new residents even with staffing shortages, oxygen & feeding deficiencies. Feeding tube bag not changed for 2 days, even though facility documented it was. Resident’s oxygen tube not changed for 4 months.

ARVADA, CO- Director admits she did not follow procedures, the facility failed to promptly address and attempt to resolve resident group complaints and grievances concerning issues of resident care and life.

DENVER, CO- Resident did not receive the anticoagulant medication between 3/3/21 (admitted ) and 3/9/21, representing 12 missed doses. Although the medication was not received from the pharmacy until 3/9/21, it was recorded on the MAR as given on several occasions.
