MESA, AZ- Facility fails to follow physician’s orders 116 times to reposition resident with pressure ulcer. Director of Nursing states “no documentation” orders had been completed and “this did not follow facility policy”.

MARYSVILLE, WA- Physician’s orders not complete, failure resulted in gangrene and amputation.

TEMPLE, TX- Resident sent to the hospital twice, family not informed.

KINGSPORT, TN- ADON stated he did not think the residents had received the proper care needed for the past couple of months. The facility’s failure resulted in psychosocial harm for Residents #1, #2, #3, #4, and #6.
