Based on observation, interview, and record review the facility failed to ensure that a resident who needed respiratory care, which included tracheostomy care and tracheal suctioning, was provided such care consistent with professional standards of practice, the comprehensive person centered care plan, the resident’s goals and preferences for 1 of 1 residents (Resident #4) reviewed for oxygen therapy.

State Findings:

The Department of Health & Human Services conducted an inspection of the facility. The following highlighted decencies listed below were found in a public survey.

Provide safe and appropriate respiratory care for a resident when needed.

Based on observation, interview, and record review the facility failed to ensure that a resident who needed respiratory care, which included tracheostomy care and tracheal suctioning, was provided such care consistent with professional standards of practice, the comprehensive person-centered care plan, the resident’s goals and preferences for 1 of 1 residents (Resident #4) reviewed for oxygen therapy.

Provide activities to meet all resident’s needs.

Based on observation, interview and record review the facility failed to provide, based on the comprehensive assessment and care plan, and the preferences of each resident, an ongoing program to support residents in their choice of activities, both facility-sponsored group and individual activities and independent activities, designed to meet the interests of and support the physical, mental, and psychosocial well-being of each resident, encouraging both independence and interaction in the community for 9 of 9 residents (Residents #1, #2, #3, #4, #5, #6, #7, #8, and #113) reviewed for activities.

-The facility failed to provide activities to Residents #1, #2, #3, #4, #5, #6, #7, #8, and #113 on an ongoing basis. These failures could place residents at risk for decline in social and mental psychosocial well-being due to the lack of ongoing activities.

Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.

Based on observation, interview, and record review, the facility failed to store, prepare, distribute, and serve food in accordance with professional standards for food service safety in 1 of 1 kitchen reviewed for food procurement in that:

-The facility failed to make sure the walk-in refrigerator was maintaining a temperature at 41 degrees Fahrenheit or below and continued to store and use food in the malfunctioning walk-in refrigerator during the time that it was not maintaining the appropriate temperature.

-The facility failed to make sure the walk-in freezer was maintaining a temperature at a level to keep frozen food solid and continued to store and use food in the malfunctioning walk-in freezer during the time that it was not maintaining the appropriate temperature.

-The facility failed to remove expired food and to discard foods according to use by date.

-The facility failed to seal, label, and date items. These failures could place residents at risk of foodborne illness and disease.
