State Findings:
Based on record review and staff interviews, the facility failed to ensure two (#29 and #183) of three of 20 sample residents reviewed for abuse were kept free from abuse.
Specifically, the facility failed to ensure Resident #183 was kept free from abuse by Resident #29.
Findings include: Record review and interviews confirmed the facility corrected the deficient practice prior to the onsite investigation on 3/22/21 to 3/25/21, resulting in the deficiency being cited at past non-compliance with a correction date of 8/26/2020.
The facility documentation from 8/5/2020 revealed Resident #183 was physically abused by Resident #29 (perpetrator). It was reported that at approximately 1:45 p.m. Resident #29 was attempting to go into Resident #183’s room. Resident #183 was on the phone with her son at the time of the altercation. Resident #183 tried two times to redirect Resident #29 from her room. Resident #29 started slapping at Resident #183 causing two skin tears to her left hand. The two residents were immediately separated. Resident #29 was placed on one-to-one staff observation.
Resident #183 was interviewed immediately following the altercation and verbalized no fear nor exhibited signs and symptoms of fear.
The facility substantiated that resident to resident abuse had occurred with an injury.
The behavior care plan, initiated 7/17/2020 and revised on 8/10/2020, revealed the resident became teary eyed when remembering the past and was started on an antidepressant.
