State Findings:
Based on record review, and interview, the facility failed to ensure that resident’s nutritional status (nutrition and hydration) was being appropriately monitored by weighing new residents once every week for the first four weeks to obtain a baseline weight, and by notifying appropriate facility staff when weight loss or a change in meal and fluid intakes had occurred for 5 (R #45, #110, #299, #356 and #358) of 5 (R #45, #110, #299, #356 and #358) residents reviewed for nutrition. This deficient practice likely resulted in weight loss in residents who were already vulnerable to losing weight due to COVID-19 (a contagious disease caused by severe acute respiratory syndrome), and caused one resident to be admitted to the hospital severely dehydrated (excessive loss of fluid from the body) and needing to be placed on life support; causing a physical and mental decline in the residents’ well being.
On 02/11/21 at 11:15 am, during an interview with R #110, he stated I was weighed at (Name of hospital) on 12/23/20 and I weighed 163. I told the speech therapist I wanted more food. I am used to being between 165-170 pounds. I don’t feel comfortable seeing my ribs and being this skinny. It’s hard being in my room all the time and I am already on anti-depressant medications. I don’t like losing weight like that. To me, it doesn’t make any sense. I’m not doing anything but sitting around in a wheel chair. Most of the time, when I open my meal tray, I say, ‘whoa, you got to be kidding me’, because I know this is not going to fill me up. I’m really not big on snacks, normally I don’t eat snacks, but I have to, to compensate for my small meals.
